Bayside Collision Center Authorization and Direction To Pay

I, the undersigned, hereby authorize Bayside Collision Center and its employees to dismantle my vehicle for damage assessment and repairs. I authorize Bayside Collision Center to drive my vehicle for the purpose of testing, repairing, and/or inspection.

I understand that Bayside Collision Center will NOT be held responsible for theft of vehicle or missing articles left in vehicle such as: ipods, ipads, cameras, sunglasses, credit cards, money, CDs', golf clubs, and etc. Please remove all items from your vehicle before or when you drop off your vehicle for repairs.

I authorize Bayside Collision Center to contact the insurance company on my behalf during the repair process.

I understand that Bayside Collision will not be responsible for any rental charges during my repairs.

I further understand that Bayside Collision Center is unable to release any vehicle without full payment.

Payment Policy

I authorize the Insurance Company to pay Bayside Collision Center direct for all supplemental and original repairs. I authorize Bayside Collision Center to have Power of Attorney to any insurance checks received for payment for repairs for my vehicle. I also agree that should any further funds owed to repair faculty be sent to me directly, I will remit the funds to Bayside Collision. Furthermore, I acknowledge that this is a legal and binding agreement.

I authorize Bayside Collision Center to move my vehicle to any of our locations as needed for repairs.

BY CLICKING SUBMIT I AM DIGITALLY SIGNING THIS FORM AND GIVING AUTHORIZATION AND POWER OF ATTORNEY TO BAYSIDE COLLISION CENTER AS STATED FORTH IN THIS DOCUMENT.

You and your employees may operate above vehicle for purposes of testing, inspection or delivery at my risk. An express mechanic's lien is acknowledged on above vehicle to secure the amount, of repairs thereto. Bayside Collision will not be held responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft, accidental or any other causes beyond your control.
